TheraSkin Purchasing

TheraSkin is available in two sizes:

• TheraSkin® Small: 1 IN x 2 IN (Item# 101TSS)
• TheraSkin® Large: 2 IN x 3 IN (Item# 102TSL)

TheraSkin® is distributed by Skin and Wound Allograft Institute (SWAI), a subsidiary of LifeNet Health.

Orders may be placed by phone (866-233-1001) and/or fax (866-884-5925). A valid purchase order number must be provided for all orders, unless otherwise approved by SWAI.

Professionals who have an account with SWAI can submit the TheraSkin Fax Order Form Only

Professionals who are ordering for the first time submit the TheraSkin Fax Order Form, Client Information Form, and Certification for the Proper Storage.  Additional ordering information can be found here.

VA Hospital Systems can order using the GSA Advantage Website:  www.gsaadvantage.com.

LIFENET HEALTH SERVICES AND SUPPORT

• SWAI Client Services is available 24 hours a day, 365 days a year.
• SWAI is open Monday – Friday, 7AM–7PM EST at (866) 233-1001.
• After hours on-call service is available for emergencies.
• All phone calls to SWAI Client Services are recorded for quality control purposes.

TheraSkin® Shipping

• TheraSkin is required to ship UPS Next Day to arrive by 10:30 am on dry ice.
• Requests for Early AM Delivery will incur a $28.50 surcharge.
• If the customer requests an alternative shipping method, additional charges may apply.

TheraSkin® Return Policy

• A Return Material Authorization number (RMA#) must be obtained from SWAI Client Services, (866)233-1001.
• Tissue must be in its unopened package. Original labeling and IFU/Package Insert must be legible and intact.
• Tissue must be maintained at proper storage requirements at all times (see package insert), and must be received by SWAI in redistributable condition.
